LTE: Is there any relationship between pre-coding matrix index and rank index ?

I know UE reports three parameters known as CQI (Channel Quality Indicator) , PMI (Pre-coding Matrix Index) and RI (Rank indicator).
Is it necessary all these three parameters should be reported and all of them have inter dependency and eNodeB considers all the parameters to take its decision ?
I am expecting answer which gives me the complete understanding of all these parameters such as
- When eNodeB configures all these parameters ?
- Can it be configured independently or together ?
- What an eNodeB does after receiving all these parameters ?

They are not parameters but feedback channels sent between UE and eNodeB.
CQI is relevant to resource allocation
PMI is relevant to Closed Loop MIMO mode precoding matrix indication
RI is relevant to Transmit Diversity or Spatial Multiplexing should be used during that TTI

If PMI reporting is there than UE will report CQI based on Ri otherwise UE will report CQI only for one codeward.like in Tm3 mod,UE will report only for one codeward while in tm4 mode, UE will report based on RI.

An UE configured for carrier aggregation, may have different radio channel conditions for primary and secondary cells.
I meant to say, primary cell may have strong signal strength at the same time UE may observed secondary cell(s) not too good.
In this case, does the same modulation and coding schemes used for primary and secondary or may use different coding and modulation schemes for different channels for the same sub frame?
