Handover is triggered for an UE from source eNodeB. The most important thing is measurement.
For detailed information of measurement stuff, please go through the 3GPP 36.331 specification.

when eNodeB receives measurement report (periodically or a-periodically) from UE, based on the measurement report, eNodeB decides whether it should initiate handover or not.
