How MME get to know which dedicated bearer should be activated for Service Request?

Based on Service request APN shall be configured at PGW. The APN have the configuration of QCI values for the bearer. This QCI values determines the dedicated bearer to create.

When source enodeB triggers Handover Required message to MME. It just has Target ID ( which just tells about the target eNodeB Id ) not at the cell level granularity . My concern is how target eNodeB figure out which of the cell out of 256 cells, need to be prepared for the received handover request message. Is there any IE carry such information within Handover Required or handover request message ?

There is a scenario where UE is having a default bearer and request for a dedicated bearer (lets say UE want to trigger a Data Call). As per my understanding:

UE sends Service Request + Bearer resource allocation.
After receiving Initial UE MSG .. the MME initiates activation of the default bearer with the S-GW/P-GW by sending the GTPv2 modify bearer request message to the S-GW.
Once MME receives the Modify Bearer response from SGW. MME will send "Bearer Resource command to SGW >>PWG.
Then PGW will trigger Create bearer request for Dedicated bearer establishment.
My query is why MME send modify bearer request to activate default bearer as already the UE was having default bearer.. Please correct me.

I have doubt in which conditions User initiated and Network initiated dedicated bearer can be established?
Any real example in LTE call.

Upon detecting Radio Link Failure UE will do RRC Connection re-establishment but to do RRC Connection re-establishment security should be activated in UE, otherwise UE will go Idle mode.So here(After detecting RLF) we are resetting all PHY,MAC,RLC,PDCP Configurations but why we are making security should be there to do RRC Connection re-establishment.
