LTE: How does an UE come to know that allocated sub carriers are different from first slot to second slot ?

Does really resource allocation happen in such a way ? When the number of allocated sub carriers may be different from first slot to second slot.

EnodeB considers CQI feedback from the UE before creating the transport block.
As per my understanding PUCCH resource allocation is not very dynamic. In this case how UE decide to use a specific MCS (Modulation and Coding Scheme) while using PUCCH channel ?

We know LTE scheduler works at the 1 millisecond level . Why it does not work at the 0.5 ms (slot) level ?
