If the UE doesn't have the autonomous gap enabled then yes measurements gaps are needed.

in case of intra frequency cells, meas gap is not required as the UEs are already tuned to serving frequency.

If two carrier frequencies are same then based on what does UE distinguish between them. Is there any UMTS kind of distinction based on a separate pilot channel or separate D/L scrambling code ?
